If we insist that verse 4 means every individual person in the world, then by the same token, in verse 1 we are commanded to make "supplications, prayers, intercessions, and thanksgivings" for every individual person in the world. It seems clear he is stating that we are to pray for people from among all types and classes (even "kings and all who are in high positions"), as God has concern for and has chosen people out of all such groups.
